Short answer
Use your clinic system's queue module or a virtual queue tool with SMS or WhatsApp updates. Messages and screens should show only the queue number, clinic and timing, never diagnoses or full names. Get opt-in, use approved utility templates, and check vendors against MOH's security essentials.
Virtual queue options for clinics
Most clinics choose between three set-ups. Which suits you depends mainly on whether your clinic management system already has a queue module.
| Option | How patients get updates | Good fit | Watch out for |
|---|---|---|---|
| Queue module in your clinic management system | SMS or app notifications sent by the system | Clinics whose system vendor offers it | Message wording and data you can't change |
| Standalone virtual queue tool | Web link, SMS or WhatsApp; patient scans a QR code to join | Clinics wanting walk-in and remote queueing fast | Double entry unless it connects to the clinic system |
| Custom queue app | WhatsApp or SMS triggered by your own rules, plus a display screen | Groups of clinics, or special flows (pharmacy, multiple doctors) | You own maintenance and security |
Whichever you pick, the patient journey is usually: join (QR code at the door, web link or front desk), get a number and an estimate, receive a 'you're next soon' message, and see the number on the display when it's their turn.
What to put in messages and on screens
Queue messages and waiting-room screens are seen by other people: family members reading a phone, other patients looking at the display. Keep them to what's needed to get the patient to the counter on time.
Include
- Queue number (for example A023)
- Clinic name and branch
- Estimated waiting time or 'X patients ahead'
- A link to check status or cancel
- First name only, if the patient chose to give it
Leave out
- Full name, NRIC or date of birth
- Reason for visit, symptoms or diagnosis
- Doctor's speciality if it reveals a condition
- Test results or medication names
- Payment amounts or insurance details
On display screens, show numbers only. A screen saying 'Mr Tan, Room 2' tells everyone in the waiting room who is visiting which doctor. If your system can't hide names on the display, switch the display to number-only mode or replace it.
SMS or WhatsApp: how WhatsApp queue messages work
WhatsApp is familiar to patients, but it has rules SMS doesn't. Meta's message templates documentation says templates are the only type of message a business can send outside a customer service window, and each template must be categorised as authentication, marketing or utility. Queue updates are utility messages.
- Customer service window: Meta's pricing page says a 24-hour window opens when a patient messages you, and resets with each new message.
- Free inside the window: Meta says utility messages sent in an open customer service window became free on 1 July 2025. If patients join the queue by sending a WhatsApp message, updates within the next 24 hours fall inside that window.
- Singapore pricing change: Meta's pricing changelog says Singapore moved out of the 'Rest of Asia Pacific' region for utility and authentication messages on 1 July 2026, with a higher rate. Check the current rate card if you send templates outside the window.
- Opt-in: you can only message patients who have opted in to receive WhatsApp messages from you, so make joining the queue the opt-in step and say so on the QR poster.
SMS is simpler and reaches every phone, but costs per message and has no read status. Many clinics offer WhatsApp with SMS as a fallback. Our WhatsApp Business API cost guide explains providers and rates in more detail.
Connecting the queue to your clinic system
A queue tool that doesn't talk to the clinic management system means front-desk staff type every patient twice. Ask these questions before you choose.
Questions for a queue vendor
MOH's Cybersecurity and Data Security Essentials include outsourcing and vendor management: understand the responsibilities set between your organisation and the vendor. Our guide to clinic cybersecurity and patient data covers the Essentials and their deadlines for clinics.
Setting up a virtual queue, step by step
This order keeps the data side right from day one.
Map your current flow
Walk-in, booked appointment, pharmacy, payment. Decide which steps get queue numbers and which don't.
Choose the tool and channel
Clinic system module, standalone tool or custom; WhatsApp with SMS fallback, or SMS only.
Write minimal-data message templates
Queue number, clinic name, wait estimate, status link. Submit WhatsApp templates as utility and wait for approval.
Set the display to numbers only
Test it from where patients sit. No names, rooms tied to specialities, or visit reasons.
Add the opt-in notice
On the QR poster and join page: what you'll send, how to stop, and a link to your privacy notice.
Train front-desk staff and keep a fallback
What to do when a patient has no phone, the system is down, or someone asks for their number to be read out.
Do it yourself or get help?
If your clinic system has a queue module, start there: switch on number-only display, edit the message wording and add an opt-in notice. A standalone queue tool is a reasonable next step for walk-in-heavy clinics. Building your own makes sense mainly for clinic groups or unusual flows. Our website cost guide covers the website side, such as a 'join the queue' page.
Our WhatsApp Notifications (S$590) sets up a WhatsApp Business API number through an official provider, submits up to 3 message templates for approval, connects them to your booking or form system and handles opt-in for PDPA. For a queue that ties into your clinic workflow with its own display screen, a Custom Business App (from S$4,900) starts with a written scope and fixed quote, and the app and data are hosted in your own account.
Frequently asked questions
Only what gets the patient to the counter: the queue number, clinic name or branch, an estimated wait or number of patients ahead, and a link to check status or cancel. Leave out full names, NRIC, date of birth, reasons for visit, diagnoses and medication. Phones are often seen by family members, so treat every message as potentially read by someone else.
Yes, through the WhatsApp Business API with patient opt-in. Messages outside a 24-hour customer service window must use pre-approved templates, and queue updates fall under the utility category. Meta says utility messages sent inside an open window have been free since 1 July 2025, so asking patients to join by sending a message keeps most updates free.
It's better not to. A display that shows names next to rooms or doctors tells everyone in the waiting room who is being seen and sometimes for what. Show queue numbers only. If your current system can't hide names on the display, change the display setting or replace the screen software before go-live.
It depends on timing. Meta doesn't charge for utility messages sent within an open customer service window, which starts when the patient messages you. Templates sent outside the window are charged per message, and Meta moved Singapore to its own, higher utility rate from 1 July 2026. Check Meta's current rate card and your provider's fees.
Whether it connects to your clinic management system, which patient data it stores and for how long, where it's hosted, who at the vendor can access it, whether the contract covers data protection and breach notification, MFA and logging for staff, number-only displays, and a fallback if the system is down. MOH's essentials include vendor management.
